Who did the pre construction termite treatment and under what license, whether the written agreement and warranty were handed over, and whether a Sub Slab Treatment Release Form was signed, which would record that parts of the slab area were never treated. That form should only be signed by the property owner, so if one exists and you did not sign it, that is a question worth asking.
At minimum: the license number and phases of the licensee with the company's full name, a foundation diagram showing whether the infestation is active or inactive, the location of visibly damaged timbers on that diagram, whether re-inspections happen and roughly how often, the conditions under which re-treatment will be done, and the total price. Every contract for controlling wood destroying organisms has to be in writing.
Not automatically, and this is where people get caught. The common arrangement in the trade is re-treatment only, which means further treatment if termites turn up during the agreement period and no obligation to repair damage. Most agreements run one year from the initial treatment. Re-treatment plus damage repair exists, is capped at a stated amount, and not every company offers it.
